Root cause analysis

 

This course teach you how to identify, prove and solve true root causes to ensure lasting results. The course follows Six Sigma's DMAIC, with main focus on Measure & Analyze phase.

Day 1

  • Structured problem solving
    • Lean Six Sigma & DMAIC
  • Define the problem
    • Problem statement
    • Thought Map & SIPOC
  • Understand current situation
    • Collect data and facts
    • Use statistics to make sense of data (center, spread and distribution)
    • Statistical Process Control (SPC)
    • Capability and problem-solving strategy
    • Set specific goals
  • Identify potential causes
    • Process mapping
    • Brainwriting, Brainstorming, Fishbone and 5 Whys

Day 2

  • Prove root causes
    • Observation
    • Turn on / off
    • Graphical analysis:
      • Histogram
      • Pareto
      • Run Chart
      • Box plot
      • Scatterplot
    • Correlation
  • Evaluate solutions and implement the best ones
  • Follow-up to ensure lasting result

Practical exercises

Galton board

We use a Galton board during the training to understand normal and special variation, basic statistics and histograms.

We will also use it for practical exercises and to generate data.

The Galton board provides a good visual understanding of what happens if you overreact to normal variation.

The exercises can be customized based on the need of the company and your own data can be used.

Webmeeting 1

You learn to use an A3 template to solve problems.

 

  • Structured problem-solving; A3, PDCA, DMAIC & 8D
  • Introduction to Lean Six Sigma
  • Review of A3 with practical assignments:
    • Describe background and set the right scope
    • Measure the current situation
    • Set a goal
    • Understand Y = f (x)
    • Understand the terms symptom, problem, and root cause
    • Brainstorm potential causes and group them in a Fishbone
    • Make causes measurable and prioritize
    • Use 5 Whys to find true root causes
    • Prove root cause using observation and turning on / off
    • Evaluate, prioritize & implement solutions
    • Follow up to ensure lasting results

Webmeeting 2

You learn how to use Control Charts to understand variation and respond correctly to measurement results.

 

  • Introduction
  • Process mapping to understand variation
  • Normal and special cause variation
  • Introduction to Control Charts
  • Normal distribution & probability
  • How to interpret a Control Chart
    • Stability
    • Sign of Special Cause variation
    • Compare control limits and customer specifications
  • Practical applications of Control Charts
  • How to determine Capability (how well the process meets customer requirements)
  • Strategies to improve Capability

Webmeeting 3

You learn fact-based techniques to prove root causes (Analyze phase of DMAIC).

 

  • Practical and statistical significance
  • Continuous and discrete data
  • Prove root causes:
    • Observation
    • Turn on/off
    • Graphical analyses:
      • Histogram
      • Pareto Chart
      • Run chart
      • Box plot
      • X-Y chart / Scatter plot

Courses can be arranged on request and customized to your business needs.

Your own data can be used in examples and exercises. 

Open courses are arranged according to the Course Calendar.

You can start e-learning when you want. You have access for a year and unlimited attempts to successfully complete multiple-choice exam.

Lunch, refreshments, and drinks during classroom training are included, and you receive a course completion certificate.
